Yes, you must update it all the time. The newer version is .126 which was out yesterday.

If it runs all the games you want to play and you're happy with it then stick with the version you've got.

Mame users tend to fall into two categories, the first use the same version of mame until something new they want to play is emulated and then they upgrade.
This method makes the update process longer and larger because there will have been more changes to the core and roms the longer you leave it between updates. It does mean that you only need to upgrade once every 6 months to a year though which kind of makes up for it if your main interest is playing the games. Personally I prefer this method as currently mame plays everything I'm interested in just fine and the few interesting games that are worth upgrading for aren't working in mame yet. When they are then I'll upgrade again.

The other category is people that upgrade Mame with each new release, keeping everything up to date. The updates are smaller, faster and easier but it does mean you're having to upgrade mame and replace roms every month.

Its worth noting that mame updates incrementally and its only worthwhile updating the full numbered versions 0.123, 0.124, 0.125 etc. They have 'u' releases every week (sometimes several times a week) but these are WIP builds between main releases and often have a lot of broken code and drivers, so avoid 0.123u1, 0.123u2 etc.
