using mame .91 need help w/ chd

i cant for the life of me get mame32 on windows xp to run or even recognize a chd file or the corresponding rom. the game is calspeed. ive tried putting the zipped rom in the roms folder and the unzipped chd file in its own folder inside the main roms folder then audit but nothing happens.please give me the long version of what to do. thanx. ps what is best version, (unofficial or official), of mame32. please include names of all pertainent plugins and how to apply them. sorry for the voluminous repuest but the tutorial is a joke.
