The first problem is that you haven't posted what emulator you're using, or what game ISO you're trying to run.

I'm having the same problem too. I'm using ePSXe 1.7.0 with Pete's D3D Driver 1.77 (because the other one is laggy).

I can run games that comes with the Disc Image File (ie Digimon World 2 and CC) but when it doesn't it doesn't work. The file comes with the ECM and the other file. I have the ImgBurn which converts the ECM to the Disc Image File. I then run that on the ePSXe and it doesn't work (ie FF7 and Threads of Fate).

Is there something missing? ANY HELLP WOULD BE GREATLY APPRECIATED!

The ecm extension can only be removed with the unecm.exe program that comes in the ecm archive. ePSXe cant run ecm files

Maybe try to start it without memcards?
