You don't say what version number of mame you are using. If its a relatively new version then try using the refreshspeed option as it sounds like a synch issue. Basically if mame is running the game at a different refresh rate to the games native refresh every now and again the video and sound go out of synch with each other causing mame to lag as it throttles the video to bring it back into line with the sound, this is a particular problem on fixed refresh displays like LCD screens. (did the problem start after changing your monitor?)
