Does it have any translations? Because it's hard to play them in Japanese.

Metal Max have English translation unfinished and judging from it's version (0.01a) the translation far from complete, but Metal Max Returns completed!

Metal Max Returns is translated by by Gideon Zhi from Aeon Genesis, which always stands for high quality translations. I would surely check this one out if I were you.

To my understanding, he do not speak japanese. He changed the ascii code to english code with someone translated the script for him and he did the rest then insert back into the rom. :)

You're right, RF. He does the hacking/inserting and others translate the scripts. It's still high quality ;)
